Without knowing the details of any clause, it's impossible to know what the club/player can or can't do.

He definitely failed one at Marseille but after discussions with several specialists, it was decided that his heart problems would not affect him unnecessarily.

You're right but sitting in a hospital in West London completing his medical means the only thing to do is actually sign on the dotted line. Medicals take place after everything has been agreed.

Which brings me to why do QPR have to honour Remy's release clause?

Suarez's contract had a release clause that stated the club needed to inform the player if an offer of over 40m was made, it didn't state they had to accept it. Remy's release clause is most likely different. It all comes down to the contract and what the clause in question actually requires the club to do.
